New Hire Forms: The following forms are used during the new hire process. The Application and Drug Policy are to be completed first (before drug test) and the New Hire packets are to be completed after the applicant’s SS check and drug test are completed.
Misc New Hire Forms: The following forms are used when employees need to update their taxes or if a manager needs the employee to sign a new form.
Warnings: Employees receive verbal warnings for their 1st and 2nd violations and written warnings for their 3rd and 4th violations of company policies. These warnings need to be written in detail and signed by both a manager and an employee. Upon completion the warning needs to be returned to HR. Below you will find directions to better assist you with completing the warnings and the form used for both verbal and written warnings.
Insurance: All full time employees are eligible for Health Insurance at the completion of their 90 days. Below you will find all the forms needed to supplement the enrollment packets sent out by the Payroll Department.
